Within the Department of Family Medicine, the Queen’s Family Health Team (QFHT) is a multidisciplinary team of health care professionals who provide primary health care services to the Kingston community. As an academic teaching centre, the Department trains future family physicians and conducts research in issues of primary health care delivery. QFHT is comprised of a number of medical practices and provides for a diverse, client centred work environment for all its employees. QFHT provides primary health services to rostered patients while modeling safe, efficient and collaborative clinical processes to Family Medicine residents and other health professional learners.
Reporting to the Nursing Manager, the incumbent will provide patient-centred primary care preventative health services to enrolled patients and/or groups of patients (i.e. smoking cessation counselling, case management and system navigation, assessment and intake of new patients, breastfeeding support, mental health supports, and support of other clinical programs as required) as part of a team of primary care health professionals. The incumbent will contribute to the education of family medicine residents/learners, providing specific skills training (eg. vaccinations, smoking cessation counselling, breastfeeding support). Registered Nurses perform a wide range of clinical skills in accordance with the Standards of Practice of the College of Nurses of Ontario as well as the policies and procedures of Queen’s Family Health Team.

KEY RESPONSIBILITIES:
- Screens and triages new patient referrals to clinical programs.
- Conducts comprehensive patient assessments (including medication reconciliation).
- Provides clinical support for patients (individually or in group settings) which may include counselling and health teaching.
- Communicates and collaborates with patients, families and all members of the health care team, as appropriate, to optimize patient outcomes.
- Is a resource person and acts as a liaison for patients, families and other members of the health care team.
- Initiates referrals to appropriate supports and services based on the patient’s identified needs.
- Communicates and collaborates with external community resources to ensure best possible care for patients.
- Provides nursing care to patients and significant others either independently or in collaboration with other members of the health care team.
- Facilitates safe and effective connections and transitions to and from needed services to support the best possible patient outcomes.
- Makes clinical decisions and exercises judgment about the care provided to patients.
- Evaluates the outcome of nursing care provided.
- Completes required documentation for each patient encounter.
- Develops, facilitates, maintains and evaluates clinical programs in accordance to established standards and QFHT Quality Plan.
- Assists in the development, maintenance and evaluation of processes, and policies that promote desired patient-centred outcomes.
- Ensures informed and voluntary consent to the nursing care and supports provided.
- Maintains privacy, confidentiality, and security of patient information.
- Embraces effective information processes and technologies that facilitate care provision and evaluation of patient-centred care.
- Fosters the establishment and continuous improvement of clinical practice to support patient outcomes that provide direction for continuity of care, and that are attainable with available resources.
- Accountable for following safe working practices.
- Undertakes other duties as assigned.
REQUIRED QUALIFICATIONS:
- Baccalaureate Degree in Nursing from an Ontario University or equivalent.
- Current Registration Certificate as a Registered Nurse in the Province of Ontario.
- BCLS Certificate.
- Minimum of three years’ experience preferably in a community and/or primary care setting.
- Case Management Experience in a clinic/community setting a must.
- Choices and Changes Workshop (recipient or willing to complete the training).
- Motivational interviewing training (recipient or willing to complete the training).
- TEACH program certification (recipient or be willing to complete the training)
SPECIAL SKILLS:
- Strong clinical skills demonstrated by knowledge and proficiency in current, evidenced-based methods and practices of primary care nursing services.
- Superior work ethic demonstrated by exceptional attendance and punctuality.
- Excellent oral and written communication skills and interpersonal skills.
- Strong initiative (solution-based).
- Ability to communicate in French considered an asset.
- Ability to work both independently and in an interdisciplinary team environment.
- Ability to prioritize, manage time effectively and be flexible in a dynamic work environment.
- Respects diversity and promotes inclusion in the workplace.
- Ability to cope in a physically and mentally demanding work environment.
- Competency in computer based programs (Electronic Medical Records, Microsoft Programs -Access, Word, Excel).
- Knowledge of relevant legislative requirements (eg. Privacy Act).
DECISION MAKING:
- Makes decisions regularly regarding patient care within the legal scope of practice of the profession, including but not limited to:
- Assessing patient needs with respect to presenting concerns and determining an appropriate plan of nursing care,
- Providing wellness care, including health screening activities such as monitoring hypertension and infant growth and development,
- Developing, facilitating and evaluating individual and group clinical programs,
- Monitoring people with stable chronic disease, such as hypertension, diabetes.
- Accountability for their decisions and actions when acting independently and in consultation and collaboration with other members of the health care team.
