Within the Department of Family Medicine, the Queen's Family Health Team (QFHT) is a multidisciplinary team of health care professionals who provide primary health care services to the Belleville community. As an academic teaching centre, the Department trains future family physicians and conducts research in issues of primary health care delivery. QFHT is comprised of a number of medical practices and provides for a diverse, client centred work environment for all its employees. QFHT provides primary health services to rostered patients while modeling safe, efficient and collaborative clinical processes to Family Medicine residents and other health professional learners.

Reporting to the Program Manager, the incumbent will provide nutrition screening, assessment, counseling and health education as a member of the interdisciplinary health team and in accordance with accepted clinical practice guidelines for Registered Dietitians. The incumbent will work collaboratively with family physicians and other members of the health care team to provide comprehensive nutritional services to patients, both individual and group, and is a resource to the clinical team. They will share their expertise and knowledge of health promotion, disease prevention and chronic disease management with QFHT patients, physicians, residents and allied health staff.

KEY RESPONSIBILITIES:

  • Conducts individual patient assessments to determine physical/nutritional status, previous management and control, & compliance with treatment. Develops individual nutritional care plan based on comprehensive assessments incorporating nutritionally related social determinants of health.
  • Provides teaching to patients explaining the relationship between their nutritional intake and medical status. Evaluates patient comprehension and compliance to a care plan.
  • Records and documents care provided in the patients' electronic medical record (health history, assessment, interventions, and plans).
  • Involves patients in decision about their own health and encourages them to take action.
  • Provides patient education through individual and/or group sessions, using a variety of teaching aids/modalities.
  • Collaborates with physicians, residents and allied health professionals. Contributes to the education of family medicine residents/learners regarding the role of the dietitian and dietary issues facing patients and families.
  • Identifies and supports access to community, volunteer and professional food services and programs appropriate to the care needs of the patient.
  • Participates in the planning, development and evaluation of treatment, education, counseling and health promotion activities of the QFHT.
  • Prepares communication resources to inform/educate patients, for example newsletter and website updates, helpful tips in menu planning, etc.
  • Maintains confidentiality in all aspects of patient, staff and organization information.
  • Maintains and develops professional competence through ongoing professional development. Participates in the Quality Assurance Program of the College of Dietitians of Ontario. Participates in self-directed learning, interdisciplinary committees and clinical audits.

REQUIRED QUALIFICATIONS

  • Bachelor of Science degree with specialty in Foods and Nutrition from a recognized University, with the completion of a dietetic internship
  • Registered Dietitian with the College of Dietitians of Ontario
  • Membership with Dietitians of Canada Association preferred
  • Minimum of 3 years experience in clinical dietetic counseling
  • Demonstrated proficiency in adult education and teaching
  • Experience in program development, implementation, monitoring and evaluation preferred.

SPECIAL SKILLS

  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care nutritional services with an emphasis on health promotion and disease prevention.
  • Excellent verbal and written communication and interpersonal skills.
  • Ability to work both independently and effectively in an interdisciplinary team environment.
  • Ability to be flexible and creative in order to develop strategies to meet the needs of a diverse patient, family and provider population.
  • Proficient in the use of computers/technology (Word, Outlook, Internet) including prior experience and knowledge of electronic medical records. 
  • Excellent organization, time management and priority setting skills.
  • Willingness to update knowledge and skills through various courses, workshops and conferences.

DECISION MAKING

  • Makes decisions regularly regarding patient care within the legal scope of practice of a Registered Dietitian.
  • Provides wellness care including nutritional screening activities, development and monitoring of treatment plans, and monitoring infant growth and nutrition.
  • Provides individual and group counseling, support and education.
  • Makes decisions and communicates with appropriate health team members to ensure transition of care.
  • Determines effective strategies for providing services, educational programs and support to patients, families, family medicine residents and students.
