POSITION: To provide social work services to individuals with developmental disabilities affiliated with Ongwanada and their families.

DUTIES:

  • Conducts psychosocial assessment, psychotherapy, and case coordination (75%):
  • Use information from individuals, families and referral sources to develop psychosocial assessments prior to the initiation of Social Work services
  • Conduct assessments by utilizing interview techniques, observation, as well as written and oral reports; Plans, organizes, and establishes services priorities based upon identified needs and modifies these priorities as needed (crisis intervention etc);
  • Provides updates regarding service planning to support teams as required;
  • Communicates and consults with other health care professionals and participates as a member of the multidisciplinary support teams regarding the psychosocial concerns of persons served
  • Attends inter-professional team meetings and person centered planning meetings as required;
  • Provides therapeutic interventions to persons served and their families (i.e. crisis response, adjustment bereavement, psychotherapy resource counselling) using the appropriate theoretical framework;
  • Provides person-centered advocacy and family education as required with systemic barriers and social injustices;
  • Develops and delivers workshops and educational groups, including Abuse Awareness, Living with Others, Internet Safety, etc;
  • Advocate and promote the interests and rights of individuals supported and their families ;
  • Available to act as liaison to the Supported Individuals Advisory Committee;
  • Provide clinical supervision to social work students and educating individuals and families about relevant topics, if supervision opportunities ae available;
  • Participate in research activities, as available;
  • Provide information to the community on Ongwanada services and other community resources;
  • Maintains competence (per the guidelines of the Ontario College of Social Workers and Social Service Workers) through in-service opportunities and continuing education;
  • Reviews eligibility criteria for referrals to oversee management of Social Work services waitlist Connect supported individuals with community resources and services, such as healthcare, housing, or financial assistance;
  • Coordinate care with other healthcare providers, schools, and organizations ;

QUALIFICATIONS, SKILLS & KNOWLEDGE REQUIRED:

  • Master of Social Work with five years direct clinical experience preferred; an equivalent combination of education and experience would be considered;
  • Current registration with the Ontario College of Social Workers and Social Service Workers;
  • Experience conducting psychosocial assessment and providing individual and family therapy;
  • Understanding of case management principles, diagnostic criteria and clinical experience in assessment formulation and report writing;
  • Experience working with individuals with developmental disabilities, including those with a dual diagnosis;
  • Comfortable with using trauma-informed approaches to counselling and psychotherapy
  • Excellent knowledge of community resources;
  • Valid G Driving License with full driving privileges, insurance and access to a reliable vehicle;
  • Excellent written and oral communication skills;
  • Familiar with use of Microsoft Office; excellent time management and organizational skills;
  • Ability to meet the physical demands of the job and attend work on a regular basis;
  • Bilingualism (French and English) an asset.
