309 Queen Mary Rd (QMR) will include a transitional housing program operated by Home Base Housing, to support up to 35 individuals who are 55+ years old with complex medical challenges. Through a provision of on-site services, the project will aim to promote wellness and social inclusion, to support the individual needs of transitional housing residents and community members.

The Community Worker is essential to the daily operations of the Transitional Housing program, providing vital programming support to create a safe, engaging, and client-centered space. Working closely with the Supervisor and Community Partners, they help ensure clients are meaningfully connected to the services and supports they need. Additionally, they assist in coordinating programming and facilitating clear communication across the system, fostering seamless collaboration between staff, clients, and service providers.

Reporting Structure: This position reports to the Supervisor or deligate when Supervisor is not present or on planned vacation. All formal performance management and disciplinary actions remain the responsibility of the Supervisor and/or Program Manager.

Duties and Responsibilities:

1. Program Planning & Engagement

  • Develop and implement low-cost, high-impact programs that support life skills development, community connection, and well-being.
  • Plan and facilitate affordable, accessible activities that align with client interests and needs.
  • Seek donations, partnerships, and creative solutions to maximize resources and expand program offerings.
  • Evaluate and adapt programming based on client feedback and emerging needs.
  • Encourage client interaction in maintaining the integrity and cleanliness of the laundry and bathroom areas after use.

2. Meal Preparation

  • Assist in ensuring Public Haelth guidelines are adhered to in communal eating areas, while ensuring it is welcoming, inclusive, and safe.
  • Plan and prepare nutritious, budget-friendly meals, considering dietary needs and food security in collaboration with clients.
  • Organize and maintain the kitchen area, ensuring food and client safety and cleanliness.
  • Encourage client participation in meal prep and life skills activities related to cooking, budgeting, and nutrition.

3. Client Engagement & Resource Navigation

  • Build trusting relationships with clients, fostering a sense of belonging and empowerment.
  • Provide one-on-one and group support, helping clients navigate services, housing, medical, and community opportunities.
  • Ensure clients have completed the necessary intakes and are connected to the appropriate supports.
  • Keep signage, pamphlets, and resource materials up to date to reflect current services and opportunities.
  • Attend all training and educational opportunities as assigned.

4. Team Collaboration & Communication

  • Work closely with the Supervisor for direction on daily tasks and priorities.
  • Act as a central point of communication between clients, and community partners.
  • Support team members with program coordination and administrative tasks as needed.
  • Assist in scheduling, booking programs, and confirming follow-ups for services and appointments.
  • Ensure all pertinent information is articulated in writing or verbally during shift change opportunities.

5. Administrative Support & Safety

  • Maintain confidential records, ensuring compliance with privacy policies.
  • Ensure safety protocols are followed, responding appropriately to crises or conflicts.
  • Answer QMR phone calls, directing inquiries as needed.
  • Perform other duties as assigned to support the overall functioning of the Transitional Housing program.

Skills, Knowledge and Education Required:

  • Knowledge of office skills and procedures
  • Knowledge of housing, healthcare and community resources in Kingston
  • Effective interpersonal skills with the ability to maintain boundaries
  • Ability to deescalate clients and provide clear firm messages
  • Excellent oral and written communication skills; interpersonal skills with ability to set and maintain boundaries
  • Strong time management skills: ability to multitask
  • Crisis management skills
  • Exceptional confidentiality awareness skills are a requirement
  • Ability to work independently and as part of an interdisciplinary team
  • Knowledge of anti-oppression and inclusion, including knowledge of supporting 2SLGBTQ+ and creating positive spaces
  • Demonstrated ability to exercise tact, diplomacy, and good judgment in responding to enquiries
  • Experience working with individuals who use substances, have mental health challenges and/or persons with complex medical needs

Qualifications:

  • Diploma or equivalent in Social Services or related field
  • Administrative experience
  • Experience working with vulnerable population
  • Excellent communication and organizational skills

Preferred:

  • Knowledge of community resources and referral systems involved with the targeted population
