The Mental Health Case Manager, reporting to the Program Manager, works as a member of a multi-disciplinary team and is responsible for providing case management services to individuals with a serious mental illness who are living in the community. This position works in close partnership with acute care hospitals and community service agencies to provide assessment, intervention, supportive counselling, service coordination, and follow-up to adults 16-years of age and older. The Case Manager is responsible for providing services to a caseload of 18-25, unless otherwise directed, on an ongoing basis. All services are provided in accordance with the mission and established protocols of the Agency.
Please note that this is a temporary position with an anticipated contract end date of March 31st, 2027.

Back on Track Facilitator:
The Back on Track Facilitator, reporting to the Program Manager, is responsible for delivering administrative and clinical services as specified in the Back on Track Local Service Provider Agreement with the Centre for Addictions and Mental Health (CAMH).
Minimum Qualifications & Experience:
- Mental Health Case Manager - Knowledge, Skills, & Abilities:
- Knowledge of and ability to apply discipline specific principles and practices to individuals who live with a serious mental illness, within a client-centered approach.
- Demonstrated in-depth knowledge of mental health symptoms, issues, psychotropic medications and associated side effects.
- Knowledge of issues related to addictions is an asset.
- Knowledge of and ability to incorporate best practices in community support for individuals living with a serious mental illness.
- Demonstrated assessment and crisis intervention skills with broad knowledge of emergency mental health and crisis stabilization.
- Demonstrated advanced clinical reasoning and decision-making skills.
- Ability to effectively utilize community resources and supports to meet the needs of individuals.
- Ability to work under pressure within a changing environment.
- Working knowledge of the Health Care Consent Act, Mental Health Act, Personal Health Information Protection Act, and other relevant legislation.
- Excellent communication and interpersonal skills to effectively engage community services and individuals, increasing awareness of services.
- Well-developed problem-solving, prioritization, and conflict resolution skills.
Back on Track Facilitator - Knowledge, Skills, & Abilities:
- Internal and external communication including registrants, Back on Track Program Managers, allied professionals, colleagues, and the general public.
- Ability to work with mandated participants.
- Computer literacy and ability to use verbal and written communication effectively.
- Ability to effectively facilitate content and process groups following a structured format and timeline.
- Ability to monitor and maintain inventory.
- Ability to establish rapport and conduct individual assessments and follow-up interviews.
- Demonstrated assessment skills and comfortable with delivering difficult information.
- Ability to plan and organize activities independently.
- High level of comfort with teleconferencing applications (e.g., Microsoft Teams, Zoom, etc.).
- Basic IT-related problem-solving skills.
Education, Training, Experience, & Other Requirements:
- College diploma in a relevant area of health study (e.g., social services, psychology, or a related field).
- 2 years of relevant experience providing services to individuals who live with a serious mental illness; experience in a community setting is preferred.
- Experience in a human service discipline with addiction-specific training is preferred.
- Experience working within a case management model is preferred.
- Equivalent combination of relevant education and experience may be considered.
- Certification as an Addictions Counsellor is preferred.
- Current certification in First Aid and CPR is an asset.
- Current Applied Suicide Intervention Skills Training (ASIST) is an asset.
- Ability to work autonomously as well as collaboratively in a multidisciplinary team environment.
- Basic proficiency in computer skills; MS Office preferred.
- Must possess a valid Ontario Driver's License, have regular access to a reliable vehicle, and provide proof of adequate vehicle insurance.
- Required to provide a satisfactory criminal record check (CRC) and vulnerable sector screening prior to hire.
- Completed CAMH Confidentiality Undertaking and Agreement and Security Clearance Check.
