RESPONSBILITIES:

  • Assesses, develops and coordinates the delivery of social, recreational programs consistent with the needs, interests and abilities of persons in attendance.
  • Respects and promotes the dignity and individuality of each person by recognizing them as individuals with unique cultural, spiritual, and psycho-social needs.
  • Participate at each program session and expedite set up and cleanup of program(s).
  • Attend in home visits that meets the needs of those unable to attend on-site programs
  • Maintaining confidential, accurate client documentation records

QUALIFICATIONS AND EXPERIENCE:

  • Diploma or degree in health care, community recreation or therapeutic recreation studies, social work, gerontology, or other related discipline
  • Experience working directly with people living with dementia and their care partners
  • A minimum of two years facilitating groups comprised of diverse individuals with complex needs, preferably in social and or recreational setting
  • History of working and supporting volunteers and/or student placements within a program environment
  • Strong knowledge of client-centred philosophy
  • Willingness to participate in professional development and work actively with team members

  • Exceptional interpersonal skills, including shared decision-making and facilitation
  • Ability to prioritize workload and manage competing tasks
  • Ability to take initiative and be resourceful
  • Excellent problem-solving and change management skills
  • Proficiency in technology (e.g.: Microsoft office and case management and care coordination systems)
  • Demonstrated ability to work independently and within a team
  • Expertise and experience in cultural sensitivity and diversity

Travel Requirements:

This position will be based out of our Kingston office, you will require access to a reliable vehicle to attend home visits throughout KFLA as needed. You will be compensated mileage while attending home visits.
